The qualification
Level 12 is an endless survival level — there is no final wave and no victory screen. It is also the qualifier.
Defeat 10,000 enemies before the 10:00 mark and Insane Mode unlocks permanently for Levels 1–11. That is the whole requirement: a kill count against a clock.
Insane is a difficulty profile applied to the levels you have already played, not a separate set of maps. Enemies, elites, bosses, spawn rates and rewards all shift together — the level layouts stay the same, so what you learned still applies. It just applies harder.
The exact multipliers are balance data and change between versions, so they are not published here. Each version's changes are recorded in the patch notes.
